Concierge medicine, private medicine and hybrid personal medicine are all terms used to describe a new style of medical care, in which patients pay an annual fee to the doctor directly for enhanced medical services not normally covered by private insurance and/or Medicare.
In the Hybrid model which we are offering to our patients, you are responsible for an annual fee. Additionally, your insurance and/or Medicare is still billed for normally covered services rendered by our practice. This is the biggest difference between the Hybrid model and the Traditional Concierge model which does not bill private insurance or Medicare. By allowing your private insurance and/or Medicare to help off-set the expense of delivering your health care, we are able to keep the annualphysician retaining fee more affordable. In exchange for the annual fee you will enjoy access to many additional services and amenities as set forth in the Medical Services Contract.
Yes. Due to the nature of the Hybrid Personal Cardiology Care Plan, where I plan to devote more time and attention to each patient who joins, membership will be limited and offered to patients on a first-come, first-serve basis. This way, I can ensure each patient who becomes a member will receive the highest level of attention and care possible. Priority will be given to established patients. If space becomes available, a new patient may be considered for acceptance into the program on a case by case basis. Dr. Bredlauâs Nurse Practitioners will be accepting new patients not enrolled in the Hybrid Personal Cardiology Care Plan.
Yes. I have done my best to make this plan affordable, I realize there will be some of you who will be unable to pay the Annual Physician Fee. If that is the case, I welcome you to remain a patient of my practice; however, your care will be primarily managed by our Nurse Practitioners under my supervision. Should you require hospitalization, I will see you in the hospital and your care would be divided between my Nurse Practitioner and myself.
Most importantly, members can be seen on the same day for an acute medical problem. We know that patients have unplanned urgencies or emergencies, so we have time allotted for that in our office schedule. If your problem is serious enough to require hospitalization, Dr. Bredlau will oversee your care in the hospital and throughout your hospital stay.
Yes. There are no limits on the number of times that you can be seen for medical conditions.
The Hybrid Personal Cardiology Care Plan will be implemented each March 1st, sign-up will begin Jan.1st of the same year.

No. Your Hybrid fee is set for the year.
Yes. Your insurance and/or Medicare will still be billed for normally covered services rendered by Dr. Bredlau and his staff. You will also need medical insurance for emergency room visits, hospitalization, specialists, prescriptions and other services.
Traditional fee-for-service plans, even if part of a preferred provider organization (PPO), will allow me to coordinate your care best. They also permit you to make appointments with specialists yourself, without PCP referral. Unlike managed care plans and some point-of-service plans, they do not require your primary cardiologist to be in their network.
Yes. You will need to pay my annual fee to be a member of the Hybrid Plan. We will bill Medicare and your secondary for medical services rendered. You will be responsible for your deductible and copay based on the laws governing Medicare.
No. But I will always prescribe less expensive generic brand medicines when deemed appropriate. Additionally, we will assist in prescription authorization.
No. I will always be available for emergency phone calls and to act as your advisor. There may be times when we agree that an emergency room visit is necessary. You are responsible for any charges incurred for services not provided by me.
If Dr. Bredlau is on vacation or out of town, members of the Hybrid Personal Cardiology Care Plan will have care provided to them by either Dr. Bredlau's Nurse Practitioner or a covering physician of Dr. Bredlauâs choosing. Your insurance and/or Medicare will be billed in the usual manner and you are responsible for any and all expenses incurred outside of your insurance, including the copay to that physician.
Yes .If there is space available. In order to provide the individualized, high-quality medical care that I have promised you, I am limiting the number of patients enrolled. If there is no space available in the Hybrid Personal Cardiology Care Plan you will be put on a waiting list and contacted immediately when an opening occurs.
If you must transfer your care, I will assist you with finding a new physician. Additionally, I will be available to correspond with your new physician regarding your health and cardiology concerns for the remainder of your membership term. (Your medical records will be sent promptly upon your release of information).
Yes. If there is availability in my practice. The annual rate will be the same for each member of the family who becomes a member.

No. Please call the office for same or next day appointments if you have an urgent medical problem.
You will be offered the option of waiting for Dr. Bredlau to return or rescheduling your appointment to a near future date or to be seen by the Nurse Practitioner.
No. Important information can get lost in translation, we must also adhere to HIPAA compliance.
Yes. You need to keep your primary care physician as this is a cardiology care plan.

Patients will be contacted by our office to review their current scheduled appointment. At that time, if you are enrolled in the Hybrid Personal Cardiology Care Plan, you will be given an appointment with Dr. Bredlau; otherwise you will be scheduled for an appointment with the Nurse Practitioner.
